Patient Information

We know that starting a new treatment journey can feel uncertain. At FitMind Therapeutics, we want every patient to feel prepared, informed, and welcomed from the very first phone call. Below is a guide to what you can expect when you become a patient at our Jacksonville office.

Before Your First Visit

When you contact us, our team will help you understand what to expect, answer any initial questions, and walk you through the steps needed to schedule your first appointment. We will ask for some basic information about your health history, current symptoms, and any treatments you have tried before.

What to Bring

For your first visit, please bring:

Your photo ID

Your insurance card

A list of current medications and dosages

A list of any previous mental health treatments, including medications and therapy

Any relevant medical records or referral notes if available

During Your Consultation

You will meet with one of our providers for a thorough conversation. We will review your treatment history, current symptoms, and goals. You will have time to ask questions and share what matters most to you. If TMS Therapy is a good fit, we will outline what your treatment plan would look like.

Confidentiality and Comfort

Our office is HIPAA compliant, and your information is kept fully confidential. Our team is committed to creating a warm, judgment free environment where you can speak openly. Many of our staff are trained in culturally sensitive care, and we offer services in English, Russian, and Ukrainian.

Helpful Tools and Resources

We offer downloadable resources to help you prepare for treatment, including the GAD 7 Anxiety Screening Tool and the Patient Health Questionnaire. These can be a starting point for understanding your symptoms before your first visit.
